GATE Forms
- Referral Form: Parents, teachers, and students may submit referrals to nominate students for further screening or assessment.
- Authorization for Assessment: Written consent is required in order to administer most individual or small-group assessments. *Please note that written authorization is NOT required for some group assessments, such as the annual administration of the OLSAT to all students in grades 3, 5, and 7.
- Appeal Form: Parents may use this form to appeal decisions by asking for the reconsideration of any part of the identification process, including assessment and placement in services.
- Consent for Services: Students who have been identified and placed in GATE services must have written parental consent on file in order to participate.

- Sample Student Data Profile - Through a rolling screening and identification process, the GATE team routinely collect a variety of student data to create a comprehensive profile of individual performance. While identification must be based on data from state-approved assessment, data from other sources is valuable for screening and placement purposes.
- Sample Written Education Plan - The Written Education Plan (WEP) documents the services that identified students receive each year, based on state definitions of service. Based on the principle that gifted students, by definition, are those who require differentiated educational experiences, the WEP reflects the connection between identified needs and related services.
